To qualify for the study you must:
- Have a preexisting diagnosis of Alzheimer's, Dementia, or Parkinson's.*
- Be wiling to participate in the study for the next 2 years.**
- Be willing/ able to travel to doctor appointments, receive treatment, and give DNA samples.
- Be willing to work with a nutritionist to adjust your diet as needed.
- Be able to sign legal documents yourself/ have a power of attorney who can do so on your behalf.
* If you do not currently have an official diagnosis but suspect that you have on of the diseases/ conditions mentioned above, you can come in to the first appointment to receive an evaluation. However, if it turns out that you do not qualify for the study then you will be expected to pay for any medical treatment received that day, including but not limited to the initial doctor's visit. Please verify pricing/ insurance eligibility with the diagnosing doctor's office BEFORE attending the appointment.
**This time period may be extended or shortened at any time without noticed based on the needs of the study. By law, you have the right to withdraw from the study at any time for any reason, or for no reason.
Registration Form
Like any study, there are potential risks in participating in the Cross DNA Study. Some of these include potential infection at the site of injection, worsening symptoms, or no change in your current condition.
Cross DNA Study does not guarantee results of any kind for those who participate in the study. By filling out this registration form, the patient and any representative(s) of the patient accept all risks associated with participating in this study.
